The application is to be filled and signed by the applicant, applicant’s attorney, applicant’s agent under a durable power of attorney, or the applicant’s legal guardian, or any other authorized representative.

Once the application is complete and submitted in the local office concerned, an eligible employee goes through the application and also the supporting documentation to forward the same for further processing.
According to the rule, an applicant is to be contacted for any doubts, but, due to the heavy workload, the applications are prone to rejection without proper intimation and hence the applicant must take care of proper documentation while applying.
The application will be processed within 45 days (90 days in case of disability to conform it) of filing it to reply about the acceptance or rejection. If the reply from the department is unsatisfactory, the applicant may file for a review within 30 days.
